How to make an awards nomination form
If you're going to give out some awards, you need to get nominations first! You can do that easily with Typeform, collecting whatever kind of data you need so your judges can make an informed decision.
This guide will show you the general idea for making a nominations form, based on what we've seen our customers do - but there's no right or wrong way to do it!
When designing your nominations, think about what information you need first. Here are some ideas of what you might want to collect:
- Contact details,
- Text answers,
- File uploads,
- Video testimonials
1. Start with a welcome screen that will encourage people to take the time to nominate for your awards:
Add a description with any important information that your respondents will need to continue.
2. Add more questions to get the information you need, like these:
A contact question, so you don't lose touch with your nominees:
A Short text question to find out what someone's job is:
A Drop-down question so nominees can choose an industry:
A Short text question to let people describe why they should be nominated:
A File upload question to collect any relevant material about your nominees:
Ask for video testimonials. These are super fun and engaging, and can really help add dynamism to your awards ceremony:
3. SaaStock uses a Yes/No question to make sure they get the right kind of nominations. They only want businesses who sell to companies with over 1,000 employees, so they put in this question:
You can use this answer in your results to filter out nominations that are not valid. An even smarter solution is to add Logic to this question. If someone answers "no" then take them to an ending that explains that you cannot accept their nomination:
4. Add another ending thanking your nominations for taking the time to apply:
Remember to edit the Button on your end screen with the text of your choice, and add a URL to guide respondents back to your website (or wherever you want them to go!):
Now you're ready to collect nominations.
