The questions you ask should depend on your process. For example, will there be a follow-up interview? Or do you want to collect everything through the questionnaire? The steps involved will determine the level of detail you need.

So, let's say you're planning a follow-up interview after the questionnaire. In this case, you might only use the survey to see if their story is worth pursuing or not—if so, best to keep it short and sweet. Just gather essential info like contact details, their role, and where they work. Plus a Long Text question where they can quickly summarize their story.
